Navigating Aged Care was created to meet the growing need for aged care navigation support in North West Tasmania, informed by Kylie’s professional experience in aged care and community services and further validated through her tertiary study findings.
At initial consultation, Navigating Aged Care engages older people in a holistic approach that supports them to enhance their health and wellbeing. By identifying and connecting with locally available government and community services, supports and programs, addressing barriers, improving accessibility and creating meaningful change through tailored navigation assistance. Providing education, practical resources and ongoing support as required.
Whether coordinating or attending health and wellbeing events or speaking with community groups or organisations, Kylie shares her passion for collaboration and connection, driving positive change through Navigating Aged Care.
Navigating Aged Care proactively upholds the Aged Care Act 2024, including the strengthened Aged Care Quality Standards and the Code of Conduct for Aged Care. Their practice is also guided by applicable federal and state legislation relevant to business operations and the health sector.
Founder, Kylie
A lifelong resident of Tasmania’s North West Coast, Kylie has built both her personal and professional life in the region, where she continues to enjoy the natural beauty and community of coastal living.
Kylie brings several years of extensive experience in the aged care sector, with a strong foundation developed during her role with the Aged Care Assessment Team (ACAT) working primarily within the My Aged Care system. Her professional background also includes coordinating residential aged care admissions and providing support for hospital discharge planning functions and community-based services in her interim role as a Social Work Assistant across both hospital and community settings.
Most recently, Kylie contributed to the Carer Gateway initiative, where she led the establishment and facilitation of peer support groups throughout North West Tasmania. During this time, she was instrumental in community engagement, promoting carer support services through event management and outreach activities.
Kylie’s completion of a Diploma in Applied Health and Community Support at the University of Tasmania further enhances her qualifications and reinforces her commitment to delivering contemporary, person-centred support. Her approach reflects current best practices in aged care and community services, underpinned by compassion, ethical professionalism and a strong focus on individual needs and self-determination.
Kylie also holds a current National Police Check and First Aid/CPR training, a Working with Vulnerable Persons Registration and is an Accredited Mental Health First Aider.
